These Terms of Service (“Terms”) govern your use of caregivingsolution.com and the application, assessment, and intake forms made available through this site (collectively, the “Site”), operated by Caregiving Solutions, LLC (“Caregiving Solutions,” “we,” “us,” or “our”). By accessing or using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

1. Who can use the Site

You must be at least 18 years old and legally able to enter into a binding agreement to use the Site or submit any application. By submitting a form, you represent that you meet these requirements and that all information you provide is true, accurate, and complete to the best of your knowledge.

2. Purpose of the Site

The Site provides information about Caregiving Solutions and allows prospective providers to submit pre-qualification, assessment, and full-application forms for Adult or Child Developmental Home (ADH/CDH) licensing in Arizona. Nothing on this Site constitutes a guarantee that your application will be accepted or that you will be approved for licensure. Licensing decisions are made by the Arizona Division of Developmental Disabilities and the Office of Licensing, Certification and Regulation, subject to applicable state and federal law.

3. Information you submit

You agree that all information you submit through our forms is true and complete to the best of your knowledge. Providing false, misleading, or incomplete information may result in denial of your application, revocation of any license obtained, and potential legal consequences. The Site’s collection and use of information is governed by our Privacy Policy.

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law principles. Any dispute arising out of or relating to these Terms or the Site will be brought exclusively in the state or federal courts located in Maricopa County, Arizona, and you consent to the jurisdiction of those courts.
