Caregiving Solutions, LLC (“Caregiving Solutions,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect when you visit caregivingsolution.com or submit one of our application or assessment forms, how we use that information, and the choices you have.
Information we collect
We collect information you voluntarily provide when you complete our forms, including:
- Contact information — name, email address, phone number, and mailing/physical address.
- Application information — information required by the Arizona Division of Developmental Disabilities (DDD) and the Office of Licensing, Certification and Regulation (OLCR) to evaluate you as a prospective Adult or Child Developmental Home (ADH/CDH) provider. This includes household composition, employment history, financial information, references, disclosure of past legal or protective-service contacts, life-safety information about your home, and narrative home-study responses.
- Sensitive information — with your consent, we collect information that is necessary for licensing, including Social Security numbers, date of birth, and disclosures regarding criminal, court, or protective-service history.
- Website usage information — basic technical data such as IP address, browser type, pages visited, and referring URL, collected through our hosting provider’s standard server logs.
How we use your information
- To evaluate your pre-qualification, assessment, and full application for caregiver licensing.
- To contact you about next steps, schedule home visits, and deliver onboarding materials.
- To prepare and submit required documents to the Arizona DDD and OLCR licensing portals on your behalf.
- To conduct background checks (APS, CPS, fingerprint clearance, Adam Walsh Act, OIG, SAM, and similar registries) where required by law.
- To comply with legal, regulatory, and contractual obligations.
How we share your information
We share your information only as necessary to provide our services or as required by law. Specifically, we may share your information with:
- Arizona government agencies — including the Department of Economic Security, Division of Developmental Disabilities, and the Office of Licensing, Certification and Regulation, for licensing, certification, and oversight purposes.
- Service providers — we use trusted third parties to help operate our website and CRM, including Netlify (website hosting), GoHighLevel (contact management and email delivery), and Google Workspace (internal communications). These providers only access your information to perform services on our behalf and are contractually obligated to protect it.
- Legal and safety purposes — where required by law, court order, subpoena, or to protect the rights, property, or safety of Caregiving Solutions, our team, the individuals we serve, or the public.
We do not sell your personal information and we do not share it with advertisers.
How we protect your information
We use industry-standard safeguards, including HTTPS encryption on all form submissions, restricted access to sensitive records, and secure third-party processors. No method of transmission over the internet is 100% secure, but we work to protect your information using reasonable administrative, technical, and physical measures.
Social Security Numbers and other sensitive identifiers
Social Security Numbers (SSNs) are collected only for household members aged 17 and older when required by Arizona DDD/OLCR background-check rules. SSNs are transmitted over an encrypted (HTTPS) connection and stored in our contact-management system (GoHighLevel) only for as long as needed to complete the background check and submit the licensing packet to the state portal. Once the licensing packet has been accepted by the state, SSNs are removed from active records consistent with our record-retention policy. Access to records containing SSNs is limited to staff members who require it to perform their duties.
How long we keep your information
We retain your application information for as long as needed to complete the licensing process and for the period required by Arizona licensing regulations and our record-retention obligations. When information is no longer required, we securely dispose of it.
Your choices
- You may request access to, correction of, or deletion of your personal information by contacting us using the details below. Some information is required for licensing compliance and cannot be deleted while your application or license is active.
- You may opt out of marketing emails at any time by using the unsubscribe link in any message or by contacting us directly. Transactional messages related to your application cannot be opted out of while the application is in progress.
Children’s privacy
This website is intended for adults applying to become licensed caregivers. We do not knowingly collect personal information directly from children under 13. If you believe a child has provided information through this site, please contact us and we will delete it.
Changes to this policy
We may update this Privacy Policy from time to time. When we do, we will revise the “Last updated” date at the top of this page. Material changes will be communicated by email where appropriate.
