How to change your DDD agency in Arizona
If you are a licensed Adult or Child Developmental Home provider and your agency is not giving you the support you need, you can move to a different one. Here is how a transfer works, step by step.

Can I change agencies?
Yes. Licensed ADH and CDH providers can transfer to another agency. There are two common timing limits to check first: transfers generally are not accepted within 90 days of your license renewal, or while you have an open amendment.
What the process looks like
- Request a call. Tell the new agency about your home and what is not working. At Caregiving Solutions, we call within one business day.
- Phone interview. You talk through your license, your home and what support you need.
- Home visit. Someone from the agency visits in person, usually for about an hour. Have your current license and the names of your support coordinators handy.
- Coaching call. You go over what comes next and get ready before paperwork starts.
- Sign the transfer paperwork. This includes the state LCR-1051A form. With us, you review and sign on your phone.
- Submission. The agency signs and submits the paperwork to the state.
- DDD approval. This usually takes about 30 days.
- Confirmed. You complete your contract and the transfer is finished.
